What Is Home Health
Care?
WHAT TO EXPECT
Home health care is skilled, physician-directed medical care delivered in the home setting. It is different from what Home Care Help provides — our caregivers deliver non-medical, personal, and companionship support. Home health care steps in when a client needs licensed clinical services such as nursing, therapy, or social work.
Home health care may be appropriate when a client:

Has recently been discharged from a hospital, rehabilitation facility, or skilled nursing facility

Is recovering from surgery, a fall, a stroke, or a significant health event

Has a chronic condition such as diabetes, heart disease, COPD, or kidney disease that requires monitoring

Needs wound care, IV therapy, or medication management by a licensed nurse

Requires physical, occupational, or speech therapy at home

Would benefit from nutritional counseling or medical social services
Home health care is typically ordered by a physician and covered by Medicare, Medi-Cal, or private insurance when medically necessary. Our team can help connect your family with the right resources to verify coverage.

Steward Home Health Agency — Provides skilled clinical services ordered by a physician — registered nursing visits, therapy sessions, wound care, health monitoring, and social services. Their licensed team coordinates directly with the client's doctor to develop and implement a personalized care plan.
When both teams are active in a client's care, communication between Home Care Help and Steward Home Health is coordinated so that nothing falls through the cracks. Families have one consistent support network — not two disconnected providers working in isolation.

GETTING STARTED

Step 1 — Talk to the physician — Home health care requires a physician's order. Ask your loved one's doctor whether home health services are appropriate, and request a referral to Steward Home Health Agency if so.

Step 2 — Contact our team — Reach out to Home Care Help and let us know that home health services are being arranged. We will coordinate with Steward's team to ensure our non-medical support schedule complements the clinical visit schedule.

Step 3 — Confirm coverage — Steward Home Health will verify insurance coverage and work with you to confirm what services are covered under Medicare, Medi-Cal, or private insurance.

Step 4 — Care begins — Steward's clinical team will conduct an initial in-home assessment and develop a personalized care plan in coordination with the physician. Home Care Help's caregivers continue their daily support alongside the clinical visits.
